ZAPROSZENIE DO SKŁADANIA OFERT

 

Lubelski Oddział Okręgowy Polskiego Czerwonego Krzyża zaprasza do składania ofert na Etap 1 remontu i termomodernizacji budynku biurowego, położonego we Włodawie przy ul. Granicznej 8A, identyfikator dz. 061901_1.0001.322/5

Przedmiotem niniejszego zaproszenia jest wyłonienie wykonawcy Etapu 1 remontu
i termomodernizacji budynku biurowego zgodnie z projektem  i załączonym przedmiarem. Etap 1 obejmuje w szczególności:

1. Prace remontowe dachu
2. Prace murarskie w tym domurowanie kominów
3. Prace tynkarskie i inne

 

Przedmiar ma  jedynie charakter pomocniczy, a potencjalny Wykonawca w cenie oferty zobowiązany jest uwzględnić wszystkie koszty niezbędne do realizacji przedmiotu zamówienia.

Termin na wykonanie: zakładany harmonogram realizacji powinien przewidywać termin zakończenia i odbioru prac nie później niż 31 grudnia 2024.

 

Oferta sporządzona na podstawie dołączonego formularza powinna zawierać:

1. Nazwę i numer NIP oferenta
2. Wydruk z CiDG/KRS jeżeli dotyczy
3. Cenę netto i brutto za całość prac remontowych wraz z cenami materiałów, transportu, wszelkimi innymi dającymi się przewidzieć kosztami
4. Termin realizacji wyrażony w dniach liczonych od daty podpisania umowy.
5. Specyfikację proponowanych urządzeń i materiałów
6. Proponowany okres gwarancji na wykonane roboty
7. Termin ważności oferty - preferowany 30 dni
8. Termin płatności - preferowany 30 dni od momentu odbioru prac
9. Potwierdzenie dokonania przelewu wadium w wysokości 10 000 zł brutto na rachunek bankowy zamawiającego nr 79 1160 2202 0000 0002 4987 5844. W tytule przelewu należy wpisać NIP oferenta oraz numer postępowania: PRC_10_2024.  Wadium podlega zwrotowi na pisemny wniosek oferenta nie wcześniej niż 7 i nie później niż 30 dni od podpisania  umowy
10. Referencje dotyczące wykonania w ciągu ostatnich 5 lat co najmniej jednej roboty remontowo-budowlanej w obiekcie kubaturowym, o wartości minimum 200 000,00 zł brutto.

LOO PCK dokona oceny ofert przyznając punkty w ramach poszczególnych kryteriów oceny. Maksymalna ilość punktów wynosi 100.

 

1. Punkty za kryterium ,,Cena" zostaną obliczone według wzoru:

1. Długość okresu gwarancji na wykonane prace, wyrażony w miesiącach- min. 24 max. 120 miesięcy. W przypadku zaoferowania minimalnej długości okresu gwarancji tj. 24 miesięcy, Wykonawca otrzyma zero (0) punktów.

W przypadku zaoferowania maksymalnej długości okresu gwarancji tj. 120 miesięcy, Wykonawca otrzyma dwadzieścia (20) punktów.

W przypadku zaoferowania gwarancji pomiędzy 24 a 120 miesięcy Wykonawca otrzyma pkt wg wzoru:

INVITATION TO SUBMISSION OF OFFERS

 

The Lublin District Branch of the Polish Red Cross invites you to submit offers for Stage 1 of the renovation and thermal modernization of the office building, located in Włodawa at ul. Graniczna 8A, ID number 061901_1.0001.322/5

The subject of this invitation is to select a contractor for Stage 1 of the renovation and thermal modernization of the office building in accordance with the design and the attached bill of quantities. Stage 1 includes in particular:

1. Roof renovation works

2. Masonry works, including chimney lining

3. Plastering and other works

 

The bill of quantities is only of an auxiliary nature, and the potential Contractor is obliged to include all costs necessary to perform the subject of the order in the price of the offer.

Deadline for completion: the planned implementation schedule should provide for the completion and acceptance of works no later than December 31, 2024.

 

The offer prepared on the basis of the attached form should include:

1. Name and Tax Identification Number of the bidder

2. Printout from CiDG/KRS, if applicable

3. Net and gross price for all renovation works together with the prices of materials, transport, any other foreseeable costs

4. Deadline for completion expressed in days counted from the date of signing the agreement.

5. Specification of proposed equipment and materials

6. Proposed warranty period for the works performed

7. Validity of the offer - preferably 30 days

8. Payment term - preferably 30 days from the date of acceptance of the works

9. Confirmation of transfer of the deposit in the amount of PLN 10,000 gross to the ordering party's bank account no. 79 1160 2202 0000 0002 4987 5844. In the transfer title, enter the bidder's NIP and the procedure number: PRC_10_2024. The deposit is subject to return upon the bidder's written request no earlier than 7 and no later than 30 days from the signing of the contract

10. References regarding the performance of at least one renovation and construction work in a cubature facility, with a value of at least PLN 200,000.00 gross, in the last 5 years. LOO PCK will evaluate the offers by awarding points within the individual evaluation criteria. The maximum number of points is 100.

 

1. Points for the "Price" criterion will be calculated according to the formula:

Cn - the lowest bid price among the non-rejected bids,

Cb - price of the bid examined.

 

2. Length of the warranty period for the work performed, expressed in months - min. 24 max. 120 months. In the event of offering the minimum warranty period, i.e. 24 months, the Contractor will receive zero (0) points.

 

In the event of offering the maximum warranty period, i.e. 120 months, the Contractor will receive twenty (20) points.

 

In the event of offering a warranty between 24 and 120 months, the Contractor will receive points according to the formula:

3. Completion date expressed in days from the date of signing the agreement calculated according to the following formula:
